Depending on your current health insurance provider or employee benefit plan, it is possible for out-of-network services to be covered in full or in part. Please contact your provider to verify this. I can provide you with the necessary documentation for you to submit for possible reimbursement from your insurance provider.
It is your responsibility to contact your insurance provider to determine your benefits and coverage prior to scheduling your first appointment with me. Here are some helpful questions you can ask your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
I currently accept all major credit cards as forms of payment. Payment must be made in full at the time of service.
I do not accept cash or checks.
If you are unable to attend an appointment, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged a fee.
Missed Appointment Policy
If you fail to attend a scheduled appointment, you will likely be charged a fee.
